Botox injections are a type of wrinkle reduction treatment that has revolutionised our approach to facial ageing, particularly in the upper face. This anti-ageing treatment works by injecting tiny amounts of botulinum toxin into specific facial muscles, which causes the muscles to relax. This reduces the appearance of the overlying wrinkles.

Contrary to popular belief, Botox does not paralyse muscles, but rather, it relaxes them.

Often done in conjunction with dermal fillers (injectable facial fillers), the art of injecting botulinum toxin lies in altering the delicate balance between the muscles of facial expression to reduce lines and wrinkles. The frown lines are one of the most commonly treated areas using Botox.

Unlike with crow’s feet, the muscles that we use to frown also depress the brow. By relaxing them and preventing that effect, we can simultaneously lift the brow, making the eyes look more open and awake.

Forehead

The forehead is another area that is commonly treated with Botox injections.

It is important to only treat a section of the forehead muscle with these anti-wrinkle injections. This leaves enough muscle movement to support the brow.

In some cases, where there is significant excess skin and heavy, hooded upper eyelids, it is best not to perform anti-wrinkle treatments on the forehead at all. Wrinkle fillers can still be considered elsewhere though.

This works especially well for women in their 30s and 40s. When we get into our 50s, however, the muscles around the eyes become very hyper-expressive. The anti-ageing options for these fine lines are twofold: increasing the dose of Botox (botulinum toxin) used, which can sometimes look unnatural, or combining the toxin with some dermal filler.

Mouth Corners

The depressor anguli ori (or DAO for short) is a muscle that goes from the corner of the mouth down towards the jaw.

Talking, eating and certain facial expressions cause it to contract, which pulls the corners of the mouth downward. Relaxing it with anti-wrinkle Botox injections has the opposite effect — it elevates the corner of the mouth. This can be quite a difficult effect to achieve and is best suited to younger patients with fine lines.

How Often Should I Have Anti-Wrinkle Treatments?

Most people find that the muscle relaxation begins to wear off at around 3 months. However…

When the muscle movement begins to increase, there is often a lag period of another month or so before the lines and wrinkles begin to re-appear. It is when the lines and wrinkles begin to re-appear that you should think about having further anti-wrinkle injections.

In the first year of a Botox treatment plan, you will most likely find that you need to have the treatment done approximately 3 times (i.e. not longer than 4 months per treatment). In the long term, however, you should aim to have the treatment no more than twice per year. This allows muscle recovery in between anti-wrinkle treatments, which minimises long term muscle atrophy.

In the very long term, after you have been using dermal fillers for a number of years, you may find you only need to have the anti-ageing treatment once per year. This is because Botox and dermal fillers promote the production of collagen and elastin, which aids in recreating that youthful appearance. Each Botox treatment will stimulate new collagen production in the skin.

Can Botox be used with other Fillers?

There are a number of fillers available over and above botulinum toxin. Of these options, dermal fillers serve the purpose of plumping the skin and restoring volume.

What Are The Side Effects & Complications?

There is minimal pain and very few side effects associated with anti-wrinkle injections — and even fewer serious complications.

Anti-wrinkle injections as part of a Botox treatment for the upper face are usually an almost pain-free procedure. The needles used are very small and, with good technique, it is almost always possible to avoid causing bruising.

One of the most common side effects of anti-wrinkle filler injections is headaches. These can affect up to 1 in 10 people. For most people, it settles down within a day or two, however, it can take up to a couple of weeks for some people. I have seen one or two patients who have found the headaches lasted the whole three-month duration of the treatment. For most people, however, the headaches are very mild and self-limiting, so it is unusual to require any type of painkiller for them.

Complications from this anti-wrinkle treatment can occur from the botulinum toxin spreading to nearby muscles that were not intended to be targeted by the treatment. In the upper face, this can cause temporary drooping of the eyelid or eyebrow. Because this usually happens as a result of a small amount of the botulinum toxin from the anti-wrinkle injections spreading to the wrong area, rather than a direct injection into the wrong area, it tends to wear off quite quickly — usually within a couple of weeks. In the lower face and neck, it is possible for the toxin to diffuse into muscles which help to move the mouth, resulting in a lopsided smile. Rarely, Botox treatment in the neck can affect speech and ability to swallow.
