What to Expect

What Happens During a Facelift Procedure?

Facelift surgery can be performed local or general anaesthetic. Most of our patients opt for local anaesthetic, to reduce the recovery time, however this is tailored to the patient preference.

Most facelifts begin the same, with the surgeon making incisions in the sideburn area (in front of the ears), curving around the back of the ear and sometimes into the hair-bearing scalp behind the ear.

What happens next however varies enormously from surgeon to surgeon, and clinic to clinic. In our practice, Dr Russell Bramhall begins work below the skin on the SMAS layer – sometimes referred to as the muscle layer. In the deep plane technique, he goes below the SMAS to release the retaining ligaments of the face so that the soft tissues can be effectively repositioned. A key component of the deep plane technique is the ability to truly reposition the tissues, rather than placing tension on them. Tension is what makes faces look tight and unnatural. Repositioning is the most effective and natural technique.

How the Results are Achieved?

The focus of our work is to release the ligaments of the face so we can truly reposition the tissues of the face. By repositioning the tissues – rather than simply placing tension on them – we are able to achieve effective rejuvenation, without the telltale signs of surgery.

Where are the Scars?

The scars are hidden in the natural crease in front of and within the ear, as well as behind this area into the hairline. In the short term, the scars can look red and raised and usually need some camouflage with hair and make up. When the incisions scars have had time to heal properly however, they become barely perceptible.

What Is The Recovery Period Like?

There will be some bruising and swelling that is part of the normal healing process, which typically gets worse before it gets better. The swelling and bruising usually peak around 2 or 3 days after surgery, before slowly starting to improve.

There are usually no stitches to remove after a facelift because we normally use dissolvable stitches.

Most people feel comfortable enough to get back to work after a couple of weeks, although hair and make up are helpful to camouflage the initial signs of surgery.

Full recovery, with the final results, typically takes several months. If you are planning your facelift to coincide with a big birthday or event, you should ideally do it 4 to 6 months before the date for optimal results.

How Long Do The Results Last?​

A common question that we are asked in the initial consultation with patients considering this treatment is, “How long do the results of a facelift last?”

The true answer to this is that the results are permanent. When the excess tissue has been removed, and the tissues repositioned, the face will always look better than if it hadn’t been done. The ageing process will, of course, continue however we recommend maintaining the outcome with some non-surgical procedures so that your facelift surgery is usually a once in a lifetime event.

What Other Procedures Can This Surgery Be Combined With?​

Most patients undergoing a facelift will also have some additional work on their neck. This can be as minor as some liposuction under the chin, to a deep neck reduction where the shape of the neck is reconstructed below the neck muscles.

Often patients having a facelift will also undergo some eyelid surgery, as well as volume replacement to restore the softness of youth as well as some laser treatments to improve the quality of the skin.

The recommended treatment plan really depends on the needs and expectations of the patient: we always advocate a patient specific approach, unique to each individual.

What Are The Potential Risks and Complications?​

Like any surgery or operation, there is a risk of experiencing side effects and complications. That is why comprehensive aftercare is essential, and comes as standard at McKeown Medical.

Thankfully, most of the complications are quite minor and self limiting. It is possible for a collection of blood or fluid to develop under the skin which needs to be drained with a needle at your follow up appointments. Much less commonly, it is possible to develop an infection that requires additional treatment with antibiotics.

On rare occasions, there can be some disfunction of the facial nerve, causing weakness on one side of the face. This is usually temporary and recovers over time. It is rare for permanent weakness to persist in the long term.

Mini Facelift vs Full Facelift

A mini-facelift is a less invasive procedure that is claimed to deliver similar results to a facelift, but with less downtime and fewer risks. It involves making the same sort of incisions as a facelift around the ears, although instead of lifting the deeper tissues of the face and neck, it simply puts a few stitches in to place tension on the face and remove some skin around the ears.

This tends to give the appearance of a nice improvement in the short term, but because the deeper tissues of the face have not been repositioned properly, the results do not last very long. Simply placing a few stitches below the skin to hold the deep tissues is a short term solution, and the tension is lost over a period of months. As the tension from a mini-facelift gives way, it can sometimes even level some unnatural pull marks on the surface of the skin.

Although the idea of a mini-facelift sounds appealing – less downtime, fewer risks – the results are usually not worth it and, even though it is less invasive, it can still produce unnatural results. For our patients who are at the stage of needing a surgical procedure, we prefer to undertake a more comprehensive procedure to really reposition the soft tissues of the face and create a result that is both natural and long lasting.
